Hi. Can somebody explain me what cause engine failure. Engine is dd s60 14l egr with 434.000 miles on it. I was going downhill and I turned Jake on. I heard little noise. Like small hummer hitting to metal. Turned Jake off and noise disappeared. Half minute later I turned Jake on again and after 20 sec white smoke came off from exhausts. Engine stopped itself. After opening the head we found that all 4 valves are broken in cyl # 3 and head has a big hole… ;-( we inspected Jake to, and there are no problems found. Before this happened engine had no problems. Engine was maintained very well... Thanks

possible valve spring broken or keeper came off and dropped a valve. once one breaks, falls into cylinder and then all chaos happens,,,,breaks all the others, damages sleeve and piston, head.. most of the time after a major break like this happens, most of the metal gets sucked back in the exhaust and intake ports and may damage other cylinders as well.

Thank you for responding desertstormboy. I have checked all springs and valve keepers. They are all in good condition and keepers are on valves.
I feel that some thing happened with jake. Looks like jake kept valves open and piston hit them. In my 15 year detroit driving experiance this kind of mystery happened first time. I realy dont know what to check and where to find the problem. I have Jake 797 in that engine.

do a thorough inspection of the the jake brake housing and all components that hold down the exhaust valve. if it is only on the #3 cylinder, then look at the housing on that perticular cylinder. was this an "all of a sudden" type failure, or have you noticed it slowly happening? what does the injector tip look like? im sure the valves banging around in there damaged it, but can you tell if maybe the injector tip possibly broke off causing the failure to the valves? was the engine "over rev'd"? going downhill what was the rpm's when the engine failed?

We reassembled Jake brake and found one of slave –pistons stock in the housing. By hummer we took it out. It was for cylinder #2. I had broken valves in cylinder # 3. We tried to find new pistons but were unable to get them. Nobody has them (dealers). I took piston to machine shop and asked them to polish it. Also polished inside of housing. Now this particular piston goes in and out like others.
Another defective “made in USA “product (Jake 797). ……and i paid more than 10 k for tow and repairs.

I would check the cam timing before you put it back together. If the timing is out it can cause the valves to hit pistons while Jaking.
