12.7 Detroit bull gear stripped

Have a 1999 12.7 60 series Detroit. Had the head replaced, cam, injectors, and cam bearings. After repairs, engine went about 6500-7000 miles before stripping teeth on the bull gear. Then replaced bull gear, idler gear, water pump gear, and crank gear. Also took head off and had head serviced to replace bent valves. After repairs engine went 700 miles before stripping the new bill gear in the exact same spot and the original one. Any thoughts as to What may be causing this?

I have put many together but have not seen this. So just guessing. Would the back gear cover been off and put on with out the alignment tool? Lack of oil but I think that take out the bull gear bearing first. Sorry but much help I am afraid.
