12v92 marine engine rpm loss under load

Hello dear friends, the 12v92 Detroit series marine main machine has the following problem.
It reaches full rpm in idle without any problem. Whenever we go on a cruise, when the transmission engages, the rpm stays at 800 rpm.
Nonsense questions come to my mind;
1. If there is a throttle problem, why does it reach full rpm at idle?
2. If there is a mechanical problem, does the idle speed of 600 rpm affect only 200 rpm?
I would be grateful if you could shed some light on this subject.
