It can be very difficult to tell, what would normally be done here is to pull the head, send it to a good machine shop to be properly cleaned and pressure tested, and magnafluxed. Nows also the time to have them check the valves, guides, seats, seals, etc. Closely inspect the old head gasket for any sort of tracking in the suspect area. Definatley change all liner o-rings also and depending on the miles you may want to go full overhaul. If the coolant is definately 100% coming from between the piston and liner you may have a cracked liner or counterbore in the block. There is a product you can get at welding supply stores called magnaflux spot check that is an aerosol spray system that can be used to find cracks not visible to the naked eye. If the block is cracked at the counterbore there is a salvage operation so you don't have to replace the block. Its also possible to get coolant in oil at the oil cooler (you would USUALLY also have oil in coolant-I've seen cold water leaks at the cooler o-rings allow coolant into the oil but not vice-versa) or at the water pump. Just be extremely thorough and you'll get the problem fixed. If you don't find anything definative in the inspection process, let us know.
good luck

I see that more often than not c15s and c13s when the head comes off. It might not be a bad idea to have it pressure tested anyways, just to pinpoint the source of the coolant.

I got a 3406E 998k miles. I'm goin thru the same problem coolant in oil. I'm going to pressure test tomorow hopefully. How much coolant was in your oil?? My oil analysis is the only thing telling me there is contamination. My oil was/is still jet black and there's no visible sign of oil in coolant. I'm just wonderin how much pressure i need to apply to the coolant to see a result if it is liner o-rings or head issue....

I put about 18psi to the radiator and I've got coolant coming down from #1 cylinder but cant see for sure since oil pump is in the way. If I decide to remove the oil pump what are the torque specs for the 3/8" bolts holding it on? Or should I assume its the #1 liner o-rings?
